Своевременная диагностика психических расстройств у лиц призывного возраста – это залог национальной безопасности. Важной задачей является прогнозирование риска расстройств личности (РЛ), которые занимают до 2/3 в структуре психических расстройств у призывников.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценить эффективность модели прогнозирования риска расстройств личности у лиц призывного возраста на основе биопсихосоциальных маркеров.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Обследовано 239 призывников в Центре психиатрии и психического здоровья им. академика В. М. Бехтерева в 2022–2024 гг. Медиана возраста – 18,0 [18,0; 19,0] лет. Экспериментальную группу (ЭГ) составили 140 (58,6 %) призывников с установленным диагнозом РЛ, группу сравнения (ГС) – 99 (41,4 %) психически здоровых призывников. Были использованы клинический (опрос, анализ жалоб и анамнестических сведений, клиническое обследование) и экспериментально-психологический методы. Экспериментально-психологическое исследование (ЭПИ) проводили с применением авторской анкеты для выявления маркеров психических расстройств у лиц призывного возраста. На основе результатов сравнения частоты встречаемости биопсихосоциальных маркеров психических расстройств в ЭГ и ГС создавалась прогнозо-аналитическая математическая модель, позволяющая оценить риск РЛ у призывников.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В ЭГ достоверно чаще (p ≤ 0,01) встречались следующие маркеры психических расстройств: отсутствие учебной деятельности, частые ссоры в семье, плохие отношения со сверстниками, буллинг сверстниками, отсутствие друзей в прошлом и в настоящий момент, ссоры в школе, совершение внезапных необдуманных поступков, вспыльчивость, способность ударить при обиде, наблюдение/лечение у психиатра, признаки Интернет-аддикции, плохое настроение, изменчивое настроение, тревога, суицидальные мысли, отрицательное отношение к военной службе. На основе выявленных маркеров была построена прогнозо-аналитическая математическая модель, позволяющая оценить риск РЛ у лиц призывного возраста. Проверка на экспериментальных данных при пороге отсечения 0,5001 показала, что точность прогноза составляет 84,1 %.</p></sec><sec><title>ОБСУЖДЕНИЕ</title><p>ОБСУЖДЕНИЕ. Включенные в математическую модель биопсихосоциальные маркеры РЛ у призывников соответствуют критериям РЛ и отражают стойкие проявления социальной дезадаптации. Среди оказывающих наибольшее влияние на результат уравнения риска переменных специфичным для призывного контингента является пункт «отношение к военной службе»; остальные маркеры соотносятся с наиболее часто встречающимися типами РЛ у призывников (эмоционально неустойчивое, зависимое, диссоциальное), а также с данными литературы.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Таким образом, выявление биопсихосоциальных маркеров РЛ позволяет успешно прогнозировать вероятность наличия психического расстройства из данной группы у лиц призывного возраста в 84,1 % случаев. Полученные сведения могут быть использованы для повышения качества диагностики РЛ при обследовании лиц призывного возраста в психиатрическом стационаре.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION. Timely diagnosis of mental disorders in conscripts is essential for national security. An important task is to predict the risk of personality disorders (PD), which account for up to two-thirds of mental disorders in conscripts.</p></sec><sec><title>OBJECTIVE</title><p>OBJECTIVE. Evaluate the efficacy of a model for predicting the risk of personality disorders in individuals of draft age based on biopsychosocial markers.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. A total of 239 conscripts were examined at the V. M. Bekhterev Center for Psychiatry and Mental Health in 2022–2024. The median age was 18.0 [18.0; 19.0] years. The experimental group (EG) consisted of 140 (58.6%) conscripts with a confirmed diagnosis of PD, and the comparison group (CG) consisted of 99 (41.4%) mentally healthy conscripts. Clinical (questionnaire, analysis of complaints and anamnestic data, clinical examination) and experimental psychological methods were used. The experimental psychological study (EPS) was conducted using a proprietary questionnaire to identify markers of mental disorders in conscripts. Based on the results of comparing the frequency of occurrence of biopsychosocial markers of mental disorders in the EG and CG, a predictive-analytical mathematical model was created to assess the risk of PD in conscripts.</p></sec><sec><title>RESULTS</title><p>RESULTS. The following markers of mental disorders were significantly more common (p ≤ 0.01) in the EG: lack of academic activity, frequent family quarrels, poor relationships with peers, bullying by peers, lack of friends in the past and present, quarrels at school, committing sudden rash acts, quick temper, ability to strike when offended, observation/treatment by a psychiatrist, signs of Internet addiction, bad mood, mood swings, anxiety, suicidal thoughts, negative attitude towards military service. Based on the identified markers, a predictive analytical mathematical model was constructed to assess the risk of PD in persons of draft age. Testing on experimental data with a cutoff threshold of 0.5001 showed that the accuracy of the prediction is 84.1%.</p></sec><sec><title>DISCUSSION</title><p>DISCUSSION. The biopsychosocial markers of PD included in the mathematical model for conscripts meet the criteria for PD and reflect persistent manifestations of social maladjustment. Among the variables specific to the conscript contingent that have the greatest influence on the outcome of the risk equation is the item “attitude toward military service”; the other markers correlate with the most common types of PD in conscripts (emotionally unstable, dependent, dissocial), as well as with data from the literature.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Thus, the identification of PD biopsychosocial markers allows to have the successful prediction of the likelihood of mental disorders from this group in individuals of draft age in 84.1% of cases.
